Complete the Online DS-160

All non-immigrant visa (except Fiancee visa) applicants applying for visas at the U.S. Embassy or Consulates in Japan are required to complete the DS-160 online application form.
If the application has not been completed fully and accurately, you will need to return with a new completed application. Please read the following instructions carefully before you complete the DS-160.
What You Need To Know:
Visa Type: Before you fill out the DS-160 online form, you need to know what visa type you are applying for as you will be required to enter additional information, according to the visa type. For details on the visa type, please visit http://www.ustraveldocs.com/jp.
Confirmation Page:
For Tokyo/Osaka/Naha, you MUST BRING a confirmation page to your interview.
For Fukuoka/Sapporo, you MUST MAIL a confirmation page one week before your interview.
The confirmation page will be provided after the DS-160 form is electronically transmitted. Please see
NOTES:
1) Enter in English: Select “Japanese” in the “Select Tooltip Language” box at the top right corner of the page. When you point the cursor to the English text, the Japanese translation will appear. Please note all questions must be completed in English.
2) Passport Book Number: For Japanese passports: If your passport is an e-passport, you do not have a Passport Book Number so click "Does not apply." If your passport is not an e-passport, then you have your Passport Book Number (10-digit number with two alphabets) at the top right of the next page of your photo attached page. Please enter the Book Number.
3) Provide Accurate and Complete Information: You will be asked to correct your application and your application may not be accepted if you answered certain questions incorrectly.
This is not limited to but includes the following questions:
- Surname
- Given Name (enter your middle name (if any) after the First Name).
- Failure to list all other names used (i.e., Maiden name, previous married name, stage name, Japanese name for non-Japanese citizen, etc.).
- Gender
- Date of birth
- Incorrect place of birth.
- Nationality
- Purpose of Travel - Visa Category.
- Omission of previous U.S. travel
- Previous U.S. travel, including to Guam/Saipan and TRANSIT OF THE U.S., must be included.)
- You must list all of your last five U.S. visits (date of arrival and length of stay). - Non-English application.
For other questions, if the answer “Does not apply” or “Do not know” is selected, but a consular officer finds this not to be the case during the interview, you may be requested to correct your application. Examples:
- Full Name in Native Language (enter in "kanji" character if you are a Japanese or Chinese citizen).
- Education information (if applicable): Provide the information on all educational institutions you have attended, not including elementary schools.
- No mailing address in Japan.
- No name or date of birth for your parents, even if they are deceased.
- No monthly salary (if applicable).
- Visa number (if applicable): The visa number is printed in red ink on the lower right of your visa.
- Third Party’s Name: No third party's name even though a third party completed the form on your behalf.
Items for multiple answers (past U.S. travel, education information, maiden name, etc.), please click "Add Another." (See figure below). More screens will appear to enter the additional information.

If you apply in SAPPORO: Select "TOKYO" for the location in the DS-160 form". Send your documents to the Consulate General SAPPORO one week before your interview.
If you apply in FUKUOKA: Select "OSAKA" for the location in the DS-160 form". Send your documents to the Consulate FUKUOKA one week before your interview.
