Replacing a Lost/Stolen Passport

If your passport is lost or stolen, you are advised to report the loss/theft to the local Japanese Police Station and obtain a police report or a report number for presentation to the Consular Officer at time of application.

Adults (age 16 and older)

• All applicants must apply in person.
• Police report (with translation) or police report number
• Present proof of U.S. Citizenship (if circumstances allow)
• Present one valid proof of ID (if circumstances allow)
• Provide one passport photo
• Complete form DS-11
• Complete form DS-64 (PDF - 67kb)
  
• Bring one self-addressed LetterPack 500 envelope (for persons using APO address, one self-addressed envelope)

Children under 16

• All applicants (children) must appear in person accompanied by both parents.
• Police report (with translation) or police report number
• Present proof of U.S. Citizenship (if circumstances allow)
• Provide one passport photo
• Complete form DS-11
• Complete form DS-64 (PDF - 67kb)
  
• Present proof of parent-child relationship (if circumstances allow)
• Parents' valid IDs
• Bring one self-addressed LetterPack 500 envelope (for persons using APO address, one self-addressed envelope)
• Submit Notarized DS-3053 (PDF - 157kb) if either parent cannot be present.

Passports for Children:

As of February 1, 2008, the two-parent consent requirement for all minors' passport services was extended to include all children under age 16. Both parents are required to be present to provide signatures at the time of application. For more information please see here.
