Checklist for Reporting the Birth of a Child Born in Japan to U.S. Citizen Parent(s)

Complete and bring the following items indicated on this checklist with the necessary forms to your appointment.

Note:

  • If you do not bring all the required documentation, we will not be able to accept your application and you will be asked to schedule another appointment at a later date.
  • You must bring the original documents and its photocopies.
  • You must provide English translations for all documents in a foreign language.

Child's Birth Certificate

  • Please bring the original "Shusshou Todoke Kisai Jiko Shomeisho" certificate , along with its English translation.
  • If one of the parents is a Japanese national, bring either the "Shusshou Todoke Kisai Jiko Shomeisho" or "Koseki Tohon" , along with its English translation. For an explanation of the Japan's "Koseki" (戸籍) Family Registry System, please see this page.

Affidavit of Child's Name

  • If you would like your child's name in the U.S. to be different from your child's Japanese name, please use this form. For example: The Japanese birth certificate might show "Tomoko Sato" but you wish the U.S. Consular Report of Birth to show "Sarah Tomoko Jones."
  • Please complete and bring the form, but do not sign.
  • Download the form here.
  • Note: All the applications should be completed with the child's U.S. name.

Parents' Marriage Certificate

  • Please bring your original government-issued Marriage Certificate.
  • If married in the United States, you need to bring the original U.S. city/county/state-issued Marriage Certificate.

Proof of termination of all prior marriages for both parents

  • Please bring original divorce decree(s) or Death Certificate(s) for all prior marriages, if applicable.

Evidence of Parent's U.S. Citizenship

Affidavit of Parentage, Physical Presence and Support (DS-5507)

  • In cases where the parents were not married at the time of the child's birth, the U.S. citizen parent must complete Form DS-5507. Do not sign the form.
  • Parents who were not married at the time of the child's birth also need to bring in evidence of their physical presence together at the time of the child's conception (i.e. passport stamps, military travel orders) as well as proof of their relationship prior to the child's birth (i.e. letters, photos).

Evidence of physical presence

  • If only one of the parents is a U.S. citizen, he/she must bring proof of physical presence in the United States prior to the child's birth. Primary evidence that may establish your physical presence in the U.S. include: Transcripts from high school and/or college, and Wage Statements, DD214 Separation Statement (for Military Members). Secondary evidence includes: Credit Card Bills, Utility bills, Tax forms, Airline ticket stubs, former/current passport showing stamps.
